Athletic Pre-Participation Physical Exams

It is an IHSAA requirement that student athletes receive a physical exam prior to the beginning of their freshman and junior years. In order to keep this cost effective for parents and convenient for students, your school’s athletic training department has developed a relationship with Licensed Physicians, Nurses, Physical therapists, and Athletic trainers in the Idaho area who will help volunteer to keep our athletes safe. For more than twenty years, we’ve come together on one day to deliver a comprehensive sports exam to our athletes.

The big bonus:

Student athletes pay only $20 for this service. The amount charged is directly donated to the high school’s athletic training departments and used to purchase educational and rehabilitation equipment, as well as supplies. Should this amount present a hardship for your family, please ask your athletic trainer for help. Please keep in mind that we do not have access to a debit or credit card machine, Only Cash or checks are accepted. Please make your check out to Meridian Athletic Training. Please put name of student/physical under the memo portion of the check.

Location: Meridian High School Library & surrounding classrooms
1900 W. Pine, Meridian, ID 83642

Time: GIRLS: 4:00– 6:00 PM. BOYS: 6:00– 8:00PM.
We encourage you to come at your schools scheduled time. No Athlete will be admitted after 7:45PM.

GIRLS Meridian high school- 4:00-6:00 PM

BOYS Meridian high school- 6:00-8:00 PM

Medical History
The history portion of the physical form & IQ (Insurance Questionnaire) form must be completed & signed by the parent or guardian prior to admission to the examination session. Please note: Additional forms such as scout or church camp forms will NOT be completed at this time.

What to wear
Student athletes must wear shorts and t-shirts.

To help minimize congestion, parents are asked to please wait for their student athletes in the hallways of MHS near the library or feel free to leave & come back later to get your Athlete. If you are not accompanying your child to this event, please be sure that they arrive with money, signed consent forms both the Physical and IQ form. Please be aware that you need to read the online concussion materials with your athlete. Then sign & initial the concussion education box on the IQ form that states, “I have read the educational site.”
